By creating an account or using FORMA (the "Service") you agree to these Terms. If you do not agree, do not use the Service. You must be at least 18 years old, or at least 13 with a parent or guardian's consent and supervision.
FORMA is software for tracking nutrition and training and for connecting clients with independent fitness coaches. FORMA is not a healthcare provider, and the Service does not provide medical advice, diagnosis or treatment.
Coaches listed on FORMA are independent professionals, not employees or agents of FORMA. FORMA does not supervise, direct or guarantee their services, qualifications, availability or results. Any agreement for coaching services is between you and the coach. Fees, if any, are set by the coach and agreed between you and the coach.
You are responsible for your account credentials and for the accuracy of what you enter. You retain ownership of content you upload (including food logs, photos and videos). You grant FORMA a limited license to store, process and display that content in order to operate the Service for you and, where you have linked a coach, for that coach.
Progress photos and check-in data are private to you and your linked coach. A coach may request permission to use a photo, your first name or your results in a testimonial. Nothing is published unless you expressly approve the specific items to be shared, and you may withdraw that approval at any time for future use.
You agree not to misuse the Service, including by attempting to access other users' data, scraping, reverse engineering, uploading unlawful content, or using the Service to harass anyone.
The Service is provided "as is" without warranties of any kind. Calorie, macronutrient, barcode and AI food-recognition data are estimates and may be inaccurate. To the fullest extent permitted by law, FORMA is not liable for indirect, incidental, special or consequential damages, or for any injury or health outcome arising from your use of the Service or from coaching services obtained through it. FORMA's total liability for any claim is limited to the amount you paid FORMA in the twelve months before the claim.
You may delete your account at any time. FORMA may suspend or terminate accounts that violate these Terms.
We may update these Terms; continued use after an update means you accept it. These Terms are governed by the laws of the State of Texas, without regard to conflict-of-law rules.
To run the Service: calculate targets, show your diary and progress, share your data with a coach you have linked, create Zoom meetings you book, and respond to support requests. We do not sell personal information and we do not use your health data for advertising.
You. A coach you have linked, who sees your logs, progress, photos and check-ins in order to coach you (you can unlink a coach at any time). Nothing is public unless you expressly approve a testimonial.
We use trusted processors to operate FORMA, under contracts that limit their use of your data: cloud database and storage hosting, an AI provider for photo food recognition, Zoom for meetings you schedule, and Open Food Facts for barcode lookups (only the barcode number is sent). Wearable data flows through the provider you connect and, where applicable, an integration service.
You can edit or delete your logs and photos, disconnect devices, unlink a coach, withdraw testimonial consent for future use, export your data, or delete your account from Settings or by contacting us. Residents of certain jurisdictions have additional rights (access, correction, deletion, portability); we honor them on request.
We keep your data while your account is active and delete or anonymize it within 30 days of account deletion, except where law requires longer. Data is encrypted in transit and at rest, and access is restricted by role.
FORMA is not directed to children under 13 and we do not knowingly collect their data.
FORMA provides general nutrition and fitness information and tools for educational purposes. It is not medical advice and is not a substitute for consultation with a physician, registered dietitian or other qualified health professional. Consult your doctor before starting any diet or exercise program, especially if you are pregnant, have a medical condition, take medication, or have a history of disordered eating. Calorie and macronutrient targets are estimates based on population formulas. AI food recognition and barcode data may be inaccurate; verify anything that matters. Stop exercising and seek medical attention if you feel pain, dizziness or shortness of breath. Results shown anywhere on FORMA are individual examples, not guarantees.
